Hockey Canada Concussion Policy and Protocol


Concussion_Recognition_Tool_5  (CRT5) This tool should be available to bench staff at all times to identify a suspected concussion. It is not intended to be used for diagnosis. Any athlete with a suspected concussion should be immediately removed from practice or play. Follow each of the steps in the CRT 5. A medical evaluation is required before an athlete can return to play, even if symptoms seem to be resolved. The Concussion Follow-up and Communication Form (Appendix 2 of the Hockey Canada Concussion Protocol)  needs to be completed and submitted to the team trainer before the athlete can return to play.

The child SCAT 5 should be used to evaluate a suspected concussion. It is intended to be used only by physicians and healthcare professionals. If a concussion is suspected, please follow the Policy and Protocol above.
